CQR Notice

CQR Notice: Downtime January 10-11

Dear Clinical Quality Reporting Customer,

We want to share with you an important downtime notice for Clinical Quality Reporting (CQR). This downtime is designed to accelerate turnaround times following some calculation requests, support PQRS-specific reporting and the download of large QRDA III files, and implement a number of other enhancements.

We expect to post release notes to the CQR App in the Product Information workspace of Podio on Monday, January 12. If you need access to Podio, please log a ticket with

Please be aware of the following:

GE will bring CQR down for a short period starting early Saturday, January 10, and expects the system to be available for full use by the morning of Sunday, January 11.

The CQR Dashboard will be unavailable early Saturday morning through that afternoon.

CQR ingestion processes will be suspended briefly for some customers during that time.

For this maintenance period, you do NOT need to turn off your CQR Channel in QIE nor your CQR subscription in the CPS or CEMR application. However, we recommend you review QIE for errors that may have occurred during the downtime or as a result of configuration changes within the CQR application.

Meaningful Use Crystal Reports v1.10.4 now available

Dear Centricity™ Customers:

The Meaningful Use Crystal Reports that support Meaningful Use 2013 criteria have been updated to support Patient Level Detail. The package includes all changes from versions 1.10, 1.10.2, and 1.10.3. It updates all Functional Measures to display Patient Level Details when running the reports in Centricity Practice Solution v12.0 and Centricity EMR v9.8. We recommend that you run these patient level detail reports and save them for audit purposes.

As a reminder, these reports are available to support the MU Flexibility rule finalized by CMS August 29, 2014, where some Eligible Professionals (EPs) may qualify to use alternate methods for attesting for Meaningful Use in 2014. Contact us at  further details on the available flexibility options.

Additional Meaningful Use Resources

We have recently released additional resources to help you make your final preparations to attest to Meaningful Use.

Service Pack 7 is now generally available. This release supports 14 additional Clinical Quality Measures (bringing the total of supported CQMs to 36), it includes PQRS updates to identify Medicare Part B patient visits, and it incorporates the MU Functional Measure changes originally available only through the Meaningful Use Update Patch.  Please email support@quatris.comand request to schedule your upgrade to 12.0.7 if you are not currently on this version.  Click here for the CPS 12.0.7 Service Pack Release Notes

GE published a revision to the Meaningful Use Workflow Info Sheet that describes new workflows and resolutions to help ensure you capture the appropriate data for calculations.

Finally, GE has released an updated version of the Meaningful Use User Guide for Centricity Practice Solution and Centricity EMR customers that will serve as your comprehensive resource for understanding how the measures are calculated and workflows that support these measures.

Product Information Notice: Clinical Visit Summary

December 23, 2014

Dear Valued Centricity Customer,

We would like to make you aware of potentially unexpected behavior when attaching a clinical visit summary to a patient’s chart that, if not immediately addressed as instructed below, has the potential in limited cases to inadvertently lead to health information from one patient being available to another patient.

Users of Centricity Practice Solution 12.0.X or Centricity EMR 9.8.X should follow the instructions in this letter to determine if you have experienced this issue in the past.

We recommend that you upgrade to Service Pack 12.0.7 to ensure you avoid the situation in the future. Please contact Quatris Support Services at  to schedule your upgrade to version 12.0.7.

Clinical Visit Summary Appending Incorrectly SPR 60572: In certain rare circumstances, a clinical visit summary (CVS) generated from within one patient’s open chart will append to another, previously-closed patient chart. This CVS could then be provided to the other patient, whether manually or automatically via secure message.

Recommended Actions:  GE has released a script that can identify the patients’ charts at risk of containing an incorrectly appended CVS. We recommend that you follow the steps in these Release Notes CVS Detection Script Release Notes to run the detection script or contact Quatris Support Services by emailing to have the script ran for you.

We recommend that you examine any at-risk charts identified by the script for incorrectly appended summaries. If any are found, we suggest you:

(1) change the incorrect CVS to Filed in Error and

(2) retract any secure messages that may have delivered the summaries.


Please note: only some of the charts identified by the script will contain an incorrectly appended summary. You will need to run the detection script monthly and manually remediate any issues until you upgrade to a version where the situation is corrected.

If you would like Quatris to assist with running this script monthly until you upgrade to version 12.0.7 a case will need to be opened with our support services team.

***A fix (patch) is available for those on Service Pack 6, and the situation is corrected in Service Pack 7.

PQRS and Service Pack 7

Physician Quality Reporting or PQRS is a reporting program that uses a combination of incentive payments and payment adjustments to promote reporting of quality information by eligible professionals. EPs satisfactorily report data on quality measures for covered Physician Fee Schedule services furnished to Medicare Part B Fee-for-Service beneficiaries. Beginning in 2015, the program also applies a payment adjustment to EPs who do not satisfactorily report data on quality measures for covered professional services.
PQRS is not Meaningful Use. While some of the requirements of the two programs align, the programs themselves are different and have separate payments and payment adjustments. African Travel Screening Protocol

In response to urgent Ebola-related recommendations by the Centers for Disease Control (CDC), GE Healthcare has published a clinical kit containing editable notifications, alerts, and clinical guidance for use with Centricity Practice Solution (CPS) versions 11.0 and higher and Centricity EMR (CEMR) versions 9.5 and higher.

This kit enables you to incorporate Ebola screening questions and clinical guidance within the Electronic Health Record.

The attached document, African Travel Screen Protocol Job Aid, outlines the required set up necessary in Centricity once the installation of the protocol has occurred. This document should be used in conjunction with the readme_AfricanTravelScreenProtocol.pdf and AfricaTravelScreeningProtocol_user_guide.pdf documents that accompany the Protocol download available on the GE Service Portal and the Quatris Podio sites. 10 Meaningful Minutes for October

Welcome to our latest installment of 5 Meaningful Minutes; our bimonthly update meant to share important and useful Meaningful Use information with you, our meaningful users. We are trying something different with this issue, click to VIEW your 5 meaningful minutes (which is actually 10 minutes, I talk slower than I write!) where I will visit with you about the hardship exception application extension, the Flexibility rule, Clinical Quality Measures, and registering for CQR. Hardship Exception Application

  • CMS has reopened the submission period for the hardship exception application for Stage 1 Year 1 EPs to avoid the 2015 Medicare payment adjustment. The Hardship Exception Application is due at 11:59 PM EST on November 30th, 2014
  • Stage 1 Year 1 EPs can and should still attest by the end of February 2015 to earn 2014 incentives and avoid 2016 Medicare penalties


Meaningful Use 2014 Flexibility Rule

  • See MU 2014 Flexibility Rule attachment for full details
  • Is the Flexibility Rule right for your practice? For most of you who have previously attested, we believe the answer is YES!
    • It is applicable for Stage 1 Year 1; Stage 1 Year 2; and Stage 2 Year 1
    • Were you using 2013 Stage 1 workflows in 2014 prior to upgrading to CPS12 or EMR9.8? If so, then the Flexibility Rule may work for you!
    • Have you “fully implemented” 2014 technology and workflows (i.e. CPS12 and all workflows, CQR and all workflows, Patient Portal and all workflows)? If not, then the Flexibility Rule may work for you!
  • How do you start on the path to the Flexibility Rule?
    • The Crystal Reports are currently available for Download on the Centricity Product Update page along with documentation about how to install them (also attached above)

Clinical Quality Measures

  • CPS12/EMR9.8 is currently ONC certified for 22 2014 Stage 1 and Stage 2 Clinical Quality Measures (CQMS). See Supported CMS Measures attachment.
    • Under the 2014 Meaningful Use guidelines, EP’s are required to report on 9 CQMs across 3 domains
    • The CPS Meaningful Use 2014 User Guide on the GE Service Portal contains specific detail on how CQR calculates CQMs, including Numerator and Denominator calculations. Download and review this document.
    • Unlike Core and Menu measures, CQMs do not have thresholds; 0% on the CQR dashboard will return a green, met response
  • GE is currently in the certification process for additional CQMs. These are scheduled to be included in the CPS12.0.7 and EMR 9.8.7 release due out in November.
    • NOTE: These CQMs are currently available in CQR, however, since they are not ONC certified and the workflows are not in the CPS Meaningful Use 2014 User Guide, the numerators are not being returned. These uncertified CQMs are unchecked upon CQR install. Do not use these CQMs until you are on CPS12.0.7 or EMR 9.8.7. A full list can be found in the CQR 1.2 release notes; some examples of these CQMs include:
      • CMS68: Documentation of Current Medications in the Medical Record
      • CMS136: ADHD Follow up Care for Children
      • CMS50: Closing the Referral Loop

Clinical Quality Reporting (CQR)

  • If you have not yet registered for CQR and have upgraded to CPS12 or EMR9.8, please (please!) do so now. Instructions on the registration process are attached.
  • CQR will be the reporting solution for 2015, so even if you are planning on taking advantage of the Flexibility rule in 2014, you need to register for CQR so you are prepared for 2015. Don’t wait for the end of the year rush; register today!
